Notes:  Bergen gives the same list on p. 344 of the "Register".  He transcribes the first name of the left column as "Wm Goulder Junr".  In the second name of the right column "Lift:" should be understood as "Left:" or more modern, "Lieut."  
Brackets indicate editorial insertions.  The blank in the title/header indicates the presence of characters or numbers which were unreadable.
